Withholding or withdrawing life-saving ventilators can become necessary when resources are insufficient. In the USA, such rationing has unique social justice dimensions. Structural elements of dominant allocation frameworks simultaneously advantage white communities, and disadvantage Black communities—who already experience a disproportionate burden of COVID-19-related job losses, hospitalisations and mortality. Using the example of New Jersey’s Crisis Standard of Care policy, we describe how dominant rationing guidance compounds for many Black patients prior unfair structural disadvantage, chiefly due to the way creatinine and (...) life expectancy are typically considered.We outline six possible policy options towards a more just approach: improving diversity in decision processes, adjusting creatinine scores, replacing creatinine, dropping creatinine, finding alternative measures, adding equity weights and rejecting the dominant model altogether. We also contrast these options with making no changes, which is not a neutral default, but in separate need of justification, despite a prominent claim that it is simply based on ‘objective medical knowledge’. In the regrettable absence of fair federal guidance, hospital and state-level policymakers should reflect on which of these, or further options, seem feasible and justifiable.Irrespective of which approach is taken, all guidance should be supplemented with a monitoring and reporting requirement on possible disparate impacts. The hope that we will be able to continue to avoid rationing ventilators must not stand in the way of revising guidance in a way that better promotes health equity and racial justice, both to be prepared, and given the significant expressive value of ventilator guidance. Bernard Williams charges that the moral psychology built into R. M. Hare’s utilitarianism is incoherent in virtue of demanding a bifurcated kind of moral thinking that is possible only for agents who fail to reflect properly on their own practical decision making. I mount a qualified defence of Hare’s view by drawing on the account of the ‘reactive attitudes’ found in P. F. Strawson’s ‘Freedom and Resentment’. Against Williams, I argue that the ‘resilience’ of the reactive attitudes ensures that our (...) taking an instrumental view of our dispositions to experience guilt and compunction, as Hare calls for us to do while engaged in ‘critical’ moral thinking, will not prevent us from experiencing these feelings as people ordinarily do while we are thinking ‘intuitively’. I also consider the implications of my argument for consequentialism more generally and (briefly) Kantianism. (shrink)

Stanovich & West distinguish between evolutionary rationality and normative rationality, and System 1 and System 2 mental processes. They hold that the main function of System 2 has to do with normative and not evolutionary rationality. We ask how System 2 could then be an adaptation, especially given S&W's own work on individual differences.

Editors and commentators ancient and modern have not responded very well to the challenge of the two riddles which round off the contest between Damoetas and Menalcas at the end of Eclogue 3 . The first is generally regarded as impossibly difficult; the second as impossibly easy. Critics take refuge in quoting Servius' despairing statement: sciendum aenigmata haec sicuti pleraque carere aperta solutione. Yet it is most unlikely that Virgil would introduce insoluble or meaningless riddles into the Bucolica. If there (...) is no solution the lines become pointless, even tasteless; and these are not epithets to be applied lightly to Virgil's poetry. (shrink)

I welcome Mr Kennedy's general approach, but query whether the concept of consumerism is so closely applicable to medical care as he maintains. However, in particular aspects, especially the handling of complaints, his criticisms echo those made by the Patients Association. Finally, I detect some ground for hope in the more enlightened attitude creeping in to the eduction of the medical student.

On 6 September 1836, George White wrote from Hatton Garden to T. B. Hall in Liverpool:I see by an advertisement that [there is] a proposition to form a Society to be called the Botanical Society of London—Its objects are the advancement of Botanical Science in general but more especially systematic and descriptive Botany—the formation of a Library, Museum & Herbarium—A meeting will be held at the Crown & Anchor, Strand, tomorrow evening & it is my intention to attend it—It has (...) been proposed thatLadiesshould be admitted!!!If the writer of those words lived up to his declared intention and did attend that or any other of the long string of inaugural meetings the Society held during the last quarter of that year, he would have been startled, perhaps even appalled to find how seriously that last-mentioned proposal had been taken. For on 3 November he would have found in the room at the Crown and Anchor Tavern (according to one report) ‘a crowded assembly of both ladies and gentlemen’. He would also have heard the founder of the Society, the nineteen-year-old Daniel Cooper, deliver a paper on the effects of light on plants, which (according to the same report) ‘excited great interest, more particularly with the ladies’. A fortnight later the meeting was again ‘numerously attended’ and again it attracted a number of the supposedly unlearned sex, some of whom by then were ‘members of the society’ unambiguously.
